We are looking for a compassionate, organized Medical Care Coordinator to join our dedicated team and help our residents bridge the gap between treatment and long-term stability.
As a Medical Care Coordinator, you are the person who ensures clients make their appointments, get their vital medications, and access critical life resources. Watch the real, first-hand progress of individuals as they rebuild their health, confidence, and independence. Key Accountabilities:
- Assist clients in accessing ongoing services, including medical, dental, vision, housing, financial assistance, and legal advocacy.
- Support clients in obtaining or reactivating disability benefits, Medicaid, and other needed resources.
- Ensure timely medication distribution and support medication compliance.
- Conduct daily medication checks and document compliance in the EMR system.
- Monitor new or updated medications to ensure appropriate documentation and continuity.
- Complete and upload all incidental funds paperwork with accuracy and adherence to agency policies.
- Assist counselors with discharge planning.
- Perform additional duties as assigned.
Essential Requirements:
- High School Diploma required, bachelor’s degree preferred.
- 1 year of Care Coordinator experience.
- 2 years of Behavioral Health experience.
- Experience working with individuals with substance use disorders preferred.
- Strong communication and problem‑solving skills.
- Ability to work effectively with a diverse population.
- Computer literacy and comfort with EMR systems.
